The purpose of this study is to evaluate the results after anatomical reduction and internal fixation with mini-acutrak screw for Os subfibulare. This study included 12 cases, which underwent anatomical reduction and acutrak screw fixation with reverse reaming technique from March 2003 to May 2005. We analysed preoperative symptoms and onset of symptoms and radiologically measured the size and amounts of displacement of Os subfibulare under inversion stress. Postoperatively we evaluated the clinical results measured by Haegawa methods and evidence of union. The onset of symptom was average 21 years(range,16-26 years). Preoperatively there were only pain around the lateral malleolus in 7 cases, and pain and instability in 5 cases. There were abnormal anterior drawer test in 2cases, abnormal inversion stress test in 2case, abnormal both test in 1 case. The postoperative clinical results were good in 10 cases, fair in 2 cases. The minimal fragment size to be reamed was above 6mm.We believe that the procedure of anatomical reduction with Acutrak screw fixation is an effective method in treating symptomatic Os subfibulare.
